This page is dedicated to providing examples of the paperwork that parents and athletes will need to complete on their Crossbar account after their athlete has been selected for a team and committed to the club. Here, you'll find clear examples of the forms, waivers, and documents required to finalize your athlete's registration for both regional and national-level teams. Please refer to these examples as a guide to ensure everything is in order as you complete the necessary steps on your Crossbar account.

Alongside all of the documents listed below, each athlete (parents) will need to provide Rush Volleyball Club with a copy of the athlete's birth certificate.
